Registering

New patient?

If you live within our practice boundary you can register as a patient by completing a registration form at reception. If you are taking repeat medication we ask you to make an appointment with one of the GPs and bring your repeat slip or medication with you. As part of the registration process, all new patients aged 45 years and over who are not on any medication need to make an appointment with the healthcare assistant for a health check. All children under the age of five will be visited at home by the health visitor, who will telephone to arrange a convenient time.

After completing the forms, please come into the practice to provide photo ID and proof of address in person.  For a child registation we’ll also need to see a birth certificate.
